NHS PATIENT DISCHARGE PROCESSES PUTTING PATIENTS AT RISK
The result of the recently published National Audit by NHS Alliance “A Very Present Danger” shows:
- The clinical care and safety of NHS patients is at risk due to a discharge process that is not fit for purpose
- 70% of GP practices say that they experience late discharge summaries ‘very often’ or ‘fairly often’
- 58% say the clinical care of patients has been compromised in the last year
- 39% report there have been instances in the past year when patient safety has been compromised
WCI is working in partnership with NHS Alliance to address the sub-standard discharge process and the adverse effect this has on continuity of care WCI is delivering electronic discharge summary solutions to a number of Trusts that are concerned about continuity of care and the safety of their patients after leaving hospital.
The NHS Alliance is the only organisation that brings together PCTs with practices, clinicians with managers and board members – and NHS primary care with its patients.
